What Self-Employed People Need to Know About Health Insurance

When you are your own boss, nobody pays part of your premium and there is no HR department to pick a plan. The good news: self-employed people have real advantages that most employees never get.

  • Tax credits are based on net income after business expenses
  • Many self-employed people can deduct their premiums
  • Your plan is yours, so it never ends when a contract does
  • You can cover your spouse and kids on the same plan
  • PPO options let you see specialists without referrals
  • Coverage can start the 1st of next month during open enrollment

Your Price Is Based on Net Income, Not Gross

Marketplace tax credits use your household income after business expenses. A lot of self-employed people overpay because they enter what they billed instead of what they kept.

We walk through your numbers with you so your estimate is right the first time, and so a good year does not surprise you at tax time.

Example: what you billed$90,000
Supplies, mileage, phone, software− $28,000
Half of self-employment tax and other adjustments− $5,000
Income used for your tax credit$57,000

Self-Employed Health Insurance FAQ

What is the best health insurance for self-employed people?

There is no single best plan. For most self-employed people the best fit is an ACA Marketplace plan if your net income qualifies for tax credits, or a private PPO if you earn above the credit limits or need a wide doctor network. We compare both side by side at no cost.

How much does health insurance cost when you're self-employed?

It depends on your age, ZIP code, household size, plan level and net income. Marketplace tax credits can lower the price a lot, and they are based on income after business expenses, not your gross sales. A free quote shows your real price in a few minutes.

Can I deduct health insurance premiums if I'm self-employed?

Many self-employed people can deduct premiums for themselves, a spouse and dependents with the self-employed health insurance deduction, as long as they are not eligible for an employer plan through a job or a spouse's job. Ask your tax preparer about your situation.

What income do I use to qualify for tax credits?

Use your expected net self-employment income for the year, which is what is left after business expenses, plus any other household income. If your income changes during the year, update it so your credit stays accurate.

When can a self-employed person enroll?

Open enrollment for 2027 coverage runs November 1, 2026 to January 15, 2027 on HealthCare.gov. Enroll by December 15 for coverage that starts January 1. Some states that run their own exchange set different dates. Outside open enrollment you need a qualifying life event, such as losing other coverage.

Can I cover my spouse and kids on the same plan?

Yes. You can put your whole household on one individual and family plan, and tax credits are based on your total household income.

I have employees. Do I need a group plan?

If you have at least one employee who is not your spouse, a small group plan may be an option, and it can help you hire and keep good people. If it is just you, or you and your spouse, an individual plan is usually the better fit.

What if my income is too high for tax credits?

You can still buy a Marketplace plan at full price, or look at private PPO plans that may offer broader networks. Retirement contributions and business expenses can lower the income that counts, so it is worth running your numbers first.
