North Carolina is a serious trucking state. I-40, I-85, and I-95 all run through it, the Port of Wilmington moves freight up the corridor, and the Raleigh, Durham, and Greensboro triangle is dense with owner-operators, small fleets, and independent drivers running regional and OTR.

If you are a driver based in or around Raleigh, you have specific options that a national article will not tell you about. Here is what coverage actually looks like in North Carolina.

Quick Answer

Truck drivers in Raleigh and across North Carolina can compare plans from Blue Cross Blue Shield NC, UnitedHealthcare, Aetna, Cigna, and Ambetter. North Carolina expanded Medicaid in 2023, so lower-income drivers may qualify; others choose an ACA or private plan. VS Health Benefits is licensed in NC and compares them, free.

What North Carolina Drivers Should Know First

North Carolina uses the federal marketplace at HealthCare.gov rather than running its own state exchange. That matters less than one recent change that matters a great deal: North Carolina expanded Medicaid in December 2023.

Before expansion, NC had a coverage gap. Drivers whose income was too low for marketplace subsidies but too high for the old Medicaid rules got nothing. That gap is now closed. If you had a slow year, if you are just getting started, or if you took time off after an injury, you may now qualify for coverage that did not exist for you before. Many drivers have not revisited this since the rules changed, and are going without coverage they now qualify for.

If you looked into coverage in North Carolina before 2024 and were told you did not qualify for anything, that answer may be out of date. It is worth checking again.

Carriers Available in the Raleigh Area

Depending on your county, drivers in and around Wake County typically have access to plans from carriers including Blue Cross Blue Shield of North Carolina, UnitedHealthcare, Aetna, Cigna, and Ambetter. Availability varies by county, and a plan offered in Wake County is not automatically offered in Johnston or Franklin.

Blue Cross Blue Shield of NC has the broadest footprint in the state and offers plans in all 100 counties, which historically has made it a common choice for drivers. Its BlueCard program also means the network travels: a BCBS plan from North Carolina is generally recognized by BCBS providers nationwide. For an OTR driver running out of Raleigh, that portability is worth real money.

Why Network Reach Matters More Here Than Almost Anywhere

A driver based in Raleigh running the I-95 corridor can be in Virginia in ninety minutes and South Carolina in two hours. Regional runs cross state lines constantly. A plan with a network limited to the Triangle is not adequate coverage for that job.

This is the mistake to avoid: a narrow-network HMO built around a local hospital system can look extremely attractive on price, and it works fine if you work in an office in Cary. For a driver, it means that a medical problem in Richmond or Florence is an out-of-network problem, and out-of-network on an HMO usually means you pay all of it.

Ask about the national network before you ask about the price.

Your RouteWhat You Need
Local, Triangle onlyA regional HMO or EPO can work and will save you money.
Regional, NC and neighboring statesA PPO, or a BCBS plan with BlueCard reciprocity. Verify multi-state coverage explicitly.
OTR, 48 statesA PPO with a genuinely national network. Do not compromise here.

What It Costs in North Carolina

Unsubsidized premiums in the Raleigh area for a single driver typically land in the range of $400 to $700 per month depending on age and plan level, with Bronze at the low end and Gold at the high end.

But most self-employed drivers do not pay that. Premium tax credits are calculated on net self-employment income after your business deductions, and for an owner-operator with a truck payment, fuel, and maintenance coming off the top, the countable income is often far lower than the gross. A driver grossing well into six figures can still qualify for a substantial credit.

And below the subsidy range, expanded Medicaid now catches drivers who previously fell through entirely.

Coverage for Small NC Fleets

If you run a small trucking operation in the Raleigh area with W-2 drivers, group coverage is a different conversation and often a better one. Group plans can be a genuine recruiting and retention tool in a market where drivers are hard to keep, and employer contributions are generally tax deductible as a business expense.

Small groups in North Carolina can typically be written with as few as one or two eligible employees depending on the carrier. If you are running a handful of trucks and losing drivers to carriers that offer benefits, this is worth pricing out. We handle group placement as well as individual coverage.

Enrolling in North Carolina

Open enrollment for 2027 coverage runs November 1 through January 15. Outside that window, you need a qualifying life event: losing coverage, moving, marriage, divorce, or a new child. You generally have 60 days.

Medicaid is different. If you qualify for expanded Medicaid in North Carolina, you can enroll at any time of year. There is no open enrollment window and no deadline. If you have been going without coverage because you assumed you missed the window, that assumption may be costing you.

Questions

North Carolina Trucker Insurance FAQ

Where can a truck driver in Raleigh get health insurance?

Drivers in the Raleigh area can buy individual coverage through the ACA marketplace, private PPO plans, or expanded Medicaid if income qualifies. Carriers commonly available in Wake County include Blue Cross Blue Shield of North Carolina, UnitedHealthcare, Aetna, Cigna, and Ambetter, though availability varies by county. VS Health Benefits is licensed in NC and compares all of them at no cost.

Did North Carolina expand Medicaid?

Yes. North Carolina expanded Medicaid in December 2023, which closed the coverage gap that previously left many low income and self-employed residents with no option. Drivers who were told before 2024 that they did not qualify for anything should check again, because the rules changed. Medicaid also has no open enrollment window, so you can apply at any time of year.

Does a North Carolina health plan cover me in other states?

It depends on the network. A PPO with a national network generally covers you across state lines. Blue Cross Blue Shield of NC plans use the BlueCard program, which is recognized by BCBS providers nationwide. A narrow local HMO built around a Triangle hospital system generally will not cover you in Virginia or South Carolina except in an emergency.

How much does health insurance cost for a truck driver in Raleigh?

Unsubsidized premiums for a single driver in the Raleigh area typically range from about $400 to $700 per month depending on age and plan level. Most self-employed drivers pay considerably less after ACA premium tax credits, which are calculated on net income after business deductions such as fuel, maintenance, and truck payments.

Can a small trucking company in North Carolina offer group health insurance?

Yes. Small group plans in North Carolina can often be written with as few as one or two eligible employees depending on the carrier. Employer contributions are generally tax deductible as a business expense, and benefits are a meaningful recruiting and retention advantage in a tight driver market.

When can I enroll in health insurance in North Carolina?

Open enrollment for 2027 coverage runs November 1 through January 15. Outside that window you need a qualifying life event such as losing coverage, moving, marriage, or a new child, and you generally have 60 days. Medicaid is the exception and can be applied for at any time.
