OSHA requires a medical evaluation before an employee wears a respirator, and the silica standard adds periodic exams for anyone above the action level for 30 or more days a year. Those are employer-paid obligations, not health plan benefits. But a crew with a real plan and a real primary care doctor clears surveillance faster and with fewer restrictions.

Exposure history makes long-term coverage worth more here

Silica, lead and asbestos exposure produce conditions that show up years later. A demolition worker with continuous coverage and a documented exposure history is in a very different position than one who has been uninsured between jobs. Crews understand this better than owners expect, which makes the benefit land.

Small crews mean a single claim is visible

A twelve-person demo company is too small for a self-funded structure to make sense, and level-funded carriers will look hard at a group this size in a high-hazard class. Fully insured community rating protects you from exactly the scenario a small high-risk group should worry about.

Licensing and prequalification increasingly ask about benefits

Abatement and structural demo licensing, plus general contractor prequalification packets, increasingly ask what you provide your employees. It is not a legal requirement, but on a competitive prequal it is another box you either check or do not.

Which structure fits

Four routes are open to a business of this size, and the right one depends on your headcount, your W-2 versus contractor mix, and how much you want to spend per head.

  • Fully insured small group — community rated, predictable, and the usual starting point from two enrolled employees up.
  • Level funded — often 10–20% below fully insured for a healthy group, with unused claims dollars refundable. Generally worth quoting from about ten enrolled employees.
  • ICHRA — reimburse individual coverage tax-free. No participation requirement, no contribution cap, and it reaches a workforce a group plan cannot.
  • QSEHRA — for employers under 50, a fixed tax-free monthly allowance, capped at $6,450 single and $13,100 family for 2026.

Does health insurance cover the OSHA respirator medical evaluation?

Usually not as a plan benefit, because it is an employment-required exam rather than treatment. OSHA requires the employer to provide it at no cost to the employee. That said, an employee with a regular doctor and controlled blood pressure clears the evaluation far more often than one being seen for the first time.

Is silica medical surveillance an employer cost or an insurance cost?

Employer. The OSHA respirable crystalline silica standard requires the employer to make exams available at no cost to employees who wear a respirator for 30 or more days a year. Your health plan is separate, and treatment for anything the surveillance uncovers is where the plan takes over.

Will a demolition classification make my health insurance more expensive?
