South Florida Small Business Health Insurance: The 2027 Guide for Miami-Dade, Broward and Palm Beach Employers

South Florida is one labor market spread across three counties. A cook in Miami Beach may live in Hialeah, a Fort Lauderdale office manager may commute from Pembroke Pines, and a Boca Raton contractor's crew may come from three different cities. That shapes how small business health insurance works here. This guide covers the rules that apply everywhere in Florida, what changes county by county, how to pick a plan type, and how to get real quotes. The short version

  • Businesses with 1 to 50 employees can buy Florida small group coverage. Carriers must accept you whatever your employees' health history.
  • There is no requirement to offer coverage under 50 full-time-equivalent employees. Offering it is a hiring and retention decision.
  • The decision that matters most here is the network, because employees often live in a different county from where they work.
  • You can start coverage on the 1st of any month. Aiming for January 1, 2027? Start in November.

The Florida Rules That Apply in All Three Counties

  • Eligibility: Florida's small group law covers 1 to 50 employees and includes self-employed owners. A one-person group can be rated up to 150% of the standard rate, so compare it against an individual ACA plan.
  • Guaranteed issue: small group plans cannot turn you down or price you higher because of an employee's medical history.
  • Participation and contribution: these are carrier rules, not state law. Carriers typically require 50% to 75% of eligible employees to enroll and often require you to pay at least 50% of employee-only premium. Employees who waive because they have other coverage usually do not count against you.
  • The November 15 to December 15 window: if you cannot meet a carrier's participation or contribution rule, federal rules let you enroll during this window anyway, with coverage starting January 1.
  • Waiting periods: you can make new hires wait for coverage, but no longer than 90 days.

The Cross-County Commute Problem

The most common mistake we fix in South Florida is a plan chosen for the office's ZIP code instead of the employees' ZIP codes. A Miami-Dade HMO can look cheap until a Broward employee finds out their nearest hospital is out of network. Three ways to handle it:

  1. Offer two plans side by side: a lower-cost HMO for people who live near the office and a PPO for those who don't. Most carriers let small groups offer more than one plan.
  2. Pick one open-access or PPO plan that works across all three counties. It costs more, and it is simpler to run.
