Small Business Health Insurance in Palm Beach County: A 2027 Guide for West Palm Beach, Boca Raton and Jupiter Employers
Palm Beach County has more than a million residents, 39 cities and towns, and thousands of small employers. Very little has been written for the ones who need group health insurance. This guide is for owners in West Palm Beach, Boca Raton, Jupiter, Delray Beach, Boynton Beach and Wellington with 1 to 50 employees. It covers who qualifies, the four real options, how Palm Beach networks differ from Miami-Dade and Broward, and how to get side-by-side quotes before your January 1 start.
Palm Beach County group health at a glance
- Who qualifies: Florida businesses with 1 to 50 employees. Florida law counts self-employed owners too.
- Start date: any 1st of the month. Many owners pick January 1 so deductibles reset with the calendar year.
- Biggest local decision: the network. North county and south county employees use different hospital systems.
- Cost to you for our help: $0. Carriers pay broker commission, and the premium is the same as buying direct.
Who Qualifies for Small Group Coverage in Palm Beach County
Florida's small group law covers employers with 1 to 50 employees. Many websites still say "2 to 50", but the statute starts at one and expressly includes self-employed individuals. A one-person group can be rated up to 150% of the standard small group rate, so a solo owner should compare a group plan against an individual ACA plan before choosing. We run both.
Headcount is only half of it. Carriers also set participation rules, typically 50% to 75% of eligible employees must enroll, and contribution rules, commonly that you pay at least half of the employee-only premium. Employees who waive because they already have coverage through a spouse, Medicare or Medicaid usually do not count against you. These rules vary by carrier, which is exactly why comparing more than one carrier matters.
There is no Florida or federal requirement for a business under 50 full-time-equivalent employees to offer coverage. Owners in Palm Beach County offer it for the same reason owners everywhere do: good employees ask for it, and it is often cheaper than losing one. Our Florida requirements guide covers the rules in detail.
Your Four Real Options
| Option | How it works | Usually fits |
|---|---|---|
| Fully insured group plan (HMO, PPO or HSA) | You pick one or more plans and pay a fixed monthly premium. The carrier carries all the claims risk. | Most groups of 2 to 50. The most familiar choice for employees. |
| Level-funded plan | Fixed monthly cost with stop-loss protection. If claims come in low, part of the money can come back to you. | Healthy groups of roughly 5 or more that want lower costs than standard rates. |
| ICHRA or QSEHRA | You give employees a tax-free allowance, and they buy their own individual plan. | Mixed-age teams, employees spread across counties, or owners who want a fixed budget. ICHRA in Florida → |
| Individual ACA plans | No group plan. Each person enrolls on their own, and some qualify for subsidies. | Solo owners and very small teams where subsidies beat what a group plan costs. |
Many Palm Beach employers mix these. For example, a group plan for W-2 staff and a subsidy review for 1099 contractors. You do not have to force everyone into one product.

Get My Free Palm Beach Quote →The Network Question: North County vs South County
This is where Palm Beach County differs most from Miami-Dade and Broward. The county is about 45 miles from Boca Raton to Jupiter, and the hospitals along that stretch belong to different systems:
- South county (Boca Raton, Delray Beach, Boynton Beach): Baptist Health runs Boca Raton Regional Hospital and Bethesda Hospital in Boynton Beach. Tenet runs Delray Medical Center and West Boca Medical Center.
- Central (West Palm Beach, Lake Worth Beach, Wellington): Tenet's St. Mary's Medical Center and Good Samaritan, HCA Florida JFK Hospital in Atlantis, HCA Florida Palms West in Loxahatchee, and Wellington Regional Medical Center.
- North county (Palm Beach Gardens, Jupiter, Tequesta): Jupiter Medical Center, an independent hospital, and Palm Beach Gardens Medical Center. Just over the county line, Cleveland Clinic Martin Health serves Stuart and much of the Treasure Coast.
The practical problem: a narrow HMO that looks great for a Boca Raton office can leave a Jupiter employee without their nearest hospital in network, and the reverse is also true. Before choosing a plan, we check each network against where your employees live, not just where the office is. Plenty of Palm Beach County employees commute in from Broward to the south or Martin and St. Lucie to the north.
Quick network rule for Palm Beach employers
If your team lives in one part of the county, a regional HMO can save real money. If they live in more than one county, look hard at a PPO or an open-access plan. If they are scattered from Broward to St. Lucie, consider an ICHRA so each person picks a plan that works where they live.
What Palm Beach Businesses Tend to Need
Plan design should follow your workforce. Here are the patterns we see most in the county:
- Professional offices in West Palm Beach and Boca Raton (law, finance, real estate, wealth management): broad PPO networks, strong specialist access, and often a richer plan for owners with a lower-cost option for staff.
- Hospitality, clubs and restaurants in Palm Beach, Delray Beach and Jupiter: variable hours and turnover. The eligibility waiting period (up to 90 days is allowed) and a low-cost base plan matter most. See our restaurant health insurance guide.
- Construction, landscaping, HVAC and marine trades: younger crews that are often split between W-2 and 1099. HSA-qualified plans and a separate path for contractors usually work best. See construction and HVAC.
- Medical and dental practices: employees who know exactly which network they want. Check before you switch carriers.
What It Costs
Florida prices small group coverage by rating area, and your business ZIP code sets yours. The same employee list can be quoted differently in West Palm Beach than in Miami. Within the county, the three biggest drivers are the ages of the people enrolling, the plan tier (Bronze through Platinum), and how broad the network is.
For context, the KFF 2025 Employer Health Benefits Survey puts the national average for employer coverage at $9,325 per year for single coverage and $26,993 for family coverage. That is total premium, before you decide how much of it the business pays. For South Florida ranges by plan tier, see our Broward County cost breakdown next door. Your Palm Beach number comes from your own census, and getting it costs nothing.
The Tax Credit Many Palm Beach Owners Miss
If you have fewer than 25 full-time-equivalent employees with modest average wages, and you pay at least half of employee-only premiums, you may qualify for the Small Business Health Care Tax Credit. It is worth up to 50% of what you contribute, for two consecutive years. It only applies to coverage bought through SHOP, so it has to be set up correctly from the start.
